ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

Location: Cobourg Yacht Club and Town of Cobourg Boardwalk

1. Assumption of Risk

Participation in this event involves physical activity. Potential risks include, but are not limited to, falls, contact with other participants and effects of weather. By signing below, the participant acknowledges these risks and voluntarily assumes all responsibility for any injury or loss that may occur.

2. Release of Liability

The participant hereby releases and holds harmless the event organizers, volunteers, sponsors, NHH Foundation and The Corporation of the Town of Cobourg from any and all claims, liabilities, or causes of action arising out of participation in this event, including those caused by negligence.

3. Medical Fitness

The participant certifies that are fit to walk, assisted or otherwise, and have not been advised otherwise by a qualified medical professional.

4. Media Consent

The participant grants permission to the organizers to use photographs, motion pictures, or recordings of the event for any legitimate promotional purpose.

5. Authorization

If the participant is under 18, a parent or legal guardian must sign on their behalf.
